We currently have a vacancy for a Senior Crisis Care Practitioner (Band 7) within the Crisis, Assessment and Treatment Team (CAHTT) in Ealing. This is a senior clinical role within a fast‑paced, multidisciplinary community mental health team that provides assessment and short‑term, home‑based treatment for individuals experiencing acute mental health crises and at risk of hospital admission. The service operates on the frontline of community mental health care and is relied upon by other services for urgent clinical support.
As a key member of CAHTT, you will work closely with the Single Point of Access (SPA), contributing to timely triage, robust gatekeeping and effective care pathways. You will hold a leadership role within the team, supporting high‑quality clinical decision‑making and positive outcomes for service users. The post operates within a 24‑hour rota, providing a comprehensive service 365 days a year, ensuring strong interfaces with partner services and consistent crisis response.
This role suits a motivated, resilient practitioner with a proactive approach, strong communication skills and a commitment to collaborative, recovery‑focused care. The post holder will provide senior clinical leadership within the Crisis Assessment and Home Treatment Team (CAHTT), overseeing the assessment, coordination and delivery of high‑quality mental health care for individuals experiencing acute mental health crises. This includes crisis assessments, community‑based interventions, pathway planning and robust gatekeeping to inpatient services and community recovery provision, with an emphasis on home treatment where appropriate.
The post holder will work closely with the Team Manager, deputising as required, and will lead on complex and high‑risk clinical situations. They will support team performance, provide clinical supervision to junior staff and contribute to staff development and service quality. The role involves active participation in service development, promoting evidence‑based practice and supporting continuous improvement within crisis pathways.
The post holder will work in close partnership with the Single Point of Access, inpatient wards, emergency services, statutory and non‑statutory agencies, carers and third‑sector organisations to support timely crisis responses and reduce unnecessary hospital admissions. The post holder will contribute to maintaining effective interfaces with inpatient services to support patient flow, reduce length of stay and ensure bed availability. The role requires participation in a 24‑hour, 7‑day rota, ensuring a responsive, safe and effective crisis service is maintained at all times.
