This role supports people with mental health conditions within new integrated mental health teams. The post holder will provide professional nursing support to primary care network–based teams, offer mental and physical health support, and help individuals engage with local communities and available resources. The position is part of a multi‑disciplinary team covering a Primary Care Network or locality and works across networks to create an innovative support structure.
The objectives of the service are:
- Consider an individual’s needs holistically – mental, physical, and social
- Reduce the need for repeated long assessments and storytelling
- Provide early advice and guidance with signposting to relevant services, supporting individuals to use community resources fully
- Bridge gaps between community mental health care, primary care, acute care, social services, and support
- Ensure easier access to the appropriate level of care such as group programmes and therapy
Key duties:
- Provide holistic and specialist bio‑psycho‑social assessment for anyone experiencing mental health difficulties and offer specialist nursing assessment where required.
- Advise primary care to improve support for patients and, where possible, meet mental health needs within a primary care setting.
- Carry out robust risk assessments, working collaboratively with patients and carers to develop a plan that supports their safety.
- Offer access to a broad range of specialist psychosocial interventions for individual patients when needed.
- Provide expert advice on pharmacological treatment options to mental health patients and non‑mental health professionals, seeking clinical guidance from the designated Consultant Psychiatrist/G.P. when specific prescribing advice is required.
