About the Role

Adult Community Nursing Service – Band 6 Community Nurses – Hounslow area, based at the Brentford Clinic, Boston Manor Road. The service operates from 0700 to 1900, seven days a week, with alternate weekends. Shifts are 0700‑1500 (early), 0830‑1630 (day) and 1100‑1900 (late).

Responsibilities

  • Assist the Community Team Manager in managing a designated caseload and ensuring holistic assessments are undertaken by competent practitioners within the nursing team.
  • Support the Locality Nursing Team Manager in delivering the locality nursing service in line with commissioned specifications, standards, activity and outcomes, within a multidisciplinary context.
  • Provide line management and leadership for the community nursing team, account for the team caseload, and develop a skilled, competent, and motivated workforce to meet patient needs.
  • Take lead for a specific area of nursing practice as a delegated responsibility, ensuring that less experienced nurses have access to best‑practice knowledge and expertise to inform and support their clinical practice.
  • Deputise in the absence of the Locality Nursing Team Manager and undertake delegated tasks as development opportunities for succession planning.
  • Work independently.
