The Integrated Community Response Service is a large multidisciplinary team made up of GPs, occupational therapists, a social worker, physiotherapists, nurses, a community matron, and support workers. The main aim of the service is to manage complex clinical cases at home, prevent unnecessary hospital admission, and support early discharge of patients following an acute admission.

Responsibilities

  • Support the team to manage a complex caseload of patients.
  • Provide leadership for the administrative element of the team.
  • Assist junior members of the multidisciplinary team.
  • Support the clinical services manager in day‑to‑day management and service development.
  • Ensure all service procedures and processes are effective and safe, enabling the service to meet contract targets and deliver a high‑quality patient experience.
