Position Overview
The admission avoidance team want to recruit a Band 7 Advanced Nurse Practitioner with a passion for caring for people in their own home and promoting independence through collaborative working. You will need to be able to identify and manage deterioration, work across inpatient and community teams and have excellent organisational skills. The Intermediate Care team is passionate about training and education and the successful candidate will be expected to provide teaching on a regular basis and will have the opportunity to receive in-house training and complete CPD courses as appropriate.
Clinical Role
The Advanced Nurse Practitioner will hold a caseload and will manage follow-up and undertake initial assessment on patients in the community. They will work closely with the MDT team to ensure safe and effective practice and that patients are seen within the required time frame.
Key Responsibilities
- Demonstrate critical thinking and act within professional boundaries to provide care for the patient, starting with initial history taking and continuing through clinical assessment, diagnosis, treatment and evaluation of care.
- Keep people who are using our services as safe as possible through sound clinical skills and effective risk assessments.
- Work autonomously in making decisions based on assessment, diagnosis and interpretation of test results.
- Ensure the best possible clinical outcomes by using up-to-date skills and adhering to evidence-based practice and relevant policies and procedures.
- Ensure the people using our services have a good experience by respecting, empowering and working in partnership with people throughout the care planning process.
