We are seeking a Band 7 0-19 Health Visiting Team Manager to be responsible for the delivery of the 0-19 service in line with the commissioned service specification, standards, activity and outcomes, in the context of multi-disciplinary locality working. The successful candidate will provide line management and leadership for a locality Health Visiting skill mix team and will be accountable for the team caseload and the development of a skilled, competent and motivated workforce, able to meet the needs of the patients on the caseload.

Key Responsibilities:

  • To be responsible for the delivery of the 0-19 service in line with the commissioned service specification, standards, activity and outcomes, in the context of multi-disciplinary locality working.
  • To provide line management and leadership for a locality 0-19 skill mix team, being accountable for the team caseload and the development of a skilled, competent and motivated workforce, able to meet the needs of the patients on the caseload.
  • To work in partnership with the early years, education, primary health care team, social services and the wider multi-disciplinary locality team, providing holistic assessment of health needs.
  • To work with the 0-19 staff, local community, other healthcare professionals and agencies in the development and delivery of services and to assess, plan, implement and evaluate individualized programmes of care in consultation with the parent/carer, young person, clinicians, allied health professionals and other members of multidisciplinary team.
  • To attend multi-professional meetings and other locality or practice-based meetings to optimise care planning and co-ordination and maintain effective working relationships with primary care, social care, education and other service providers.
