Tonbridge PCN is seeking a Band 5 or Band 6 Registered Nurse to join our expanding Single Neighbourhood Team, delivering high-quality, patient-centred care alongside a multidisciplinary group including Paramedics, Clinical Pharmacists, ANPs and Care Coordinators.
The role focuses on frailty care, CGAs, housebound and care home visits, vaccinations, and nurse-led clinics, with opportunities to mentor staff and develop clinical skills within primary care.
