Care Coordinator - Tonbridge Primary Care Network
West Kent Primary Care is looking to recruit a Care Coordinator to support Tonbridge Primary Care Network.
We are seeking an enthusiastic and organised Care Coordinator to join our Primary Care Network (PCN) multidisciplinary team (MDT). The post holder will work proactively across all member practices to coordinate personalised care for our patient population, with a particular focus on individuals with complex health and care needs.
The post holder will mainly be based at Trenchwood Medical Centre, TN10 3ET.
Main duties of the job
The role will have a particular focus on supporting People living with frailty,Care home residents,Housebound patients,Patients requiring proactive care,Individuals at risk of hospital admission,Patients requiring coordinated care across multiple health and care services.
The Care Coordinator will act as a central point of contact for patients, carers, GP practices and partner organisations, improving continuity of care by coordinating services across primary care, community services, secondary care, adult social care and the voluntary and community sector.

Job responsibilities
Using population health intelligence and risk stratification tools, the post holder will proactively identify patients who would benefit from personalised care and support planning, helping people navigate health and care services, promoting independence and self-management, reducing health inequalities and supporting the delivery of the PCN's proactive care, Enhanced Health in Care Homes (EHCH) and frailty programmes.
In addition, the Care Coordinator will support the operational delivery of PCN hub services by coordinating and maintaining clinic schedules, managing patient bookings, cancellations and rebooking where required, and providing administrative support for vaccination programmes and other NHS and PCN clinics.
Person Specification
Qualifications
- Educated to GCSE / A Level
- NVQ in Healthcare or Business Administration (or relevant experience)
- Ongoing internal and external training to keep up to date with changes/ developments
Professional skills
- Skills to communicate on complex information
- Ability to pull together comprehensive process documents
- Negotiating, networking and persuasive skills
- Ability to communicate effectively at a range of levels
- Problem solving skills and ability to respond to sudden unexpected demands
- Excellent time management skills with the ability to re-prioritise
- Skills for manipulating information
Experience
- Experience of working with healthcare professionals and or previous experience in the NHS or social care or relevant field
- Able to demonstrate a clear understanding of working with confidential information and an understanding of service user confidentiality
- Working in a multi-disciplinary setting
- Vulnerable adults awareness
- Experience working with elderly patients in hospital or in the community
- Experience in end of life care
- Knowledge/familiarity with medical terminology
- Experience of looking after care home residents
- Experience of administrative duties
- Working in a busy and demanding environment whilst delivering in a timely manner
- Understanding of current issues facing the NHS
- Knowledge or use of EMIS
