We are seeking an enthusiastic and motivated Nursing Associate or newly qualified Nursing Associate to join our expanding Single Neighbourhood Team within the PCN. This is an exciting opportunity to work alongside a multidisciplinary team that includes Paramedics, Clinical Pharmacists, Advanced Nurse Practitioners, and Care Coordinators, supporting the delivery of high‑quality, patient‑centred care across our member practices.

Responsibilities:

  • Managing and supporting our frailty cohort, including palliative care patients, housebound individuals, and those living with frailty.
  • Contributing to the delivery of Comprehensive Geriatric Assessments (CGAs) and coordinating ongoing care and treatment with GPs and the multidisciplinary team.
  • Delivering a range of nurse‑led clinics and undertaking housebound and care home visits.
  • Providing vaccinations and, where appropriately trained, procedures such as FeNO testing, ECGs, ear irrigation, and other interventions.

Qualifications:

  • Nursing Associate or newly qualified Nursing Associate.
  • Completed trainee nurse associate training or already qualified.
