West Kent Primary Care is recruiting a Frailty Pharmacist to support the Tunbridge Wells Primary Care Network in expanding its frailty and care homes team. You will conduct medication reviews in homes, in patients’ homes, and practices, partnering with care home staff to improve safety and outcomes.
The role offers independence as a practitioner with GP collaboration, and training support to pursue CPPE pathway and Independent Prescriber courses; a full driving licence is beneficial.
