West Kent Primary Care is seeking a Nursing Associate to join the Tonbridge PCN within the wider PCN network. The role involves managing frailty cohorts, delivering CGAs, and coordinating care with GPs and other healthcare professionals in a multi-disciplinary setting across Tonbridge practices.
The successful candidate will undertake nurse-led clinics, housebound and care home visits, vaccinations, and, where trained, procedures such as FeNO testing and ECGs, contributing to integrated care.
