West Kent Primary Care (WKPC) is seeking to employ an Advanced Nurse Practitioner (ANP) on behalf of Mote & Wallis Primary Care Network (PCN) to work across their 3 member practices. This role will support The Mote Medical Practice and Wallis Avenue Surgery in expanding capacity for patients to access a clinical professional. The practices have a combined patient population of around 17,000. The post holder will work across three sites alongside the PCN Lead Pharmacist and PCN Pharmacy Technician. The PCN also provides MSK and Children’s Social Prescribing services.

Main duties of the job:

  • The ANP is a caring, compassionate, and experienced nurse who works within their professional boundaries and scope of practice within the primary care setting.
  • The post holder applies advanced assessment and critical thinking skills to decision-making when treating patients following initial history taking and clinical assessment to determine a diagnosis, treatment, and treatment plan.
  • The Advanced Nurse Practitioner will prioritise and triage the needs of patients making any necessary referrals for investigations in line with local policies.
  • The post holder will be required to communicate and work collaboratively with all stakeholders to meet the needs of patients, supporting the delivery of policy and procedures, and providing nurse leadership as required.
  • They will focus on the four pillars of advanced practice: Clinical Practice, Leadership, Education, and Research.

Job responsibilities:

  • Work as part of a multi-disciplinary team to deliver safe and high quality care.
  • Carry out clinical nursing practice at an advanced level using expert clinical skills, critical judgements & knowledge to deliver appropriate and safe care.
  • Assess, diagnose, plan, implement and evaluate treatment/interventions and care for patients presenting with acute, long term and complex health needs.
  • Triage and treat patients, performing assessment of their needs and makes referrals as necessary to other members of the primary and secondary health care teams as appropriate.
  • Interpret investigation findings to determine effective treatment plans and discuss with GPs where a second opinion is required.
  • Proactively identify, diagnose and manage treatment plans for patients at risk of developing a long-term condition, using both non-pharmacological and pharmacological treatments.
  • Co-ordinate the planning and delivery of care ensuring patients and carers or relatives are involved where required.
  • Prescribe and review medication that is appropriate for patient needs and in accordance with latest evidence-based practice, national and practice protocols and within the scope of qualified practice.
  • Diagnose and manage acute and chronic conditions, including drug and non-drug based treatment methods into a management plan.
  • Request diagnostic investigations and interpret within agreed protocols e.g. x-ray, blood tests, scans, etc.
  • Assess, diagnose and treat patients in practice premises or their own home who require medical attention and/or refer to appropriate agency.
  • Refer patients to other members of the multi-disciplinary practice teams, secondary care organisations and other organisations as necessary.
