An exciting opportunity has come up at West End Clinic for a Practice Nurse.
The practice list size is approximately 4,500 patients.
Our team consists of 1 partners, 2 salaried GPs, 2 practice nurses, and 1 healthcare assistant.
Main duties of the job
We are looking for a proactive, reliable and forward-thinking practice nurse to join our team.
The successful candidate will form part of the nursing team in providing nursing and health promotion services to the Practice population.
The candidate will provide holistic care to patients with or without direct referral from a doctor or senior nurse, within clinical guidelines.
The Practice Nurse will be responsible for several clinical areas such as COPD and Diabetes as well as supporting the practice management team in the reviewing and delivery of clinical policy and procedure.

- Job responsibilities
- To assess, plan, implement and evaluate patient care
- To be involved in immunisation, including campaigns as required by the public health needs of the Practice population
- To exercise knowledge and skills in chronic disease preferably trained in Diabetes Management, wound assessment and management, contraception, and other clinical skills as appropriate to the role
- To participate in infection control
- To participate in the screening and health promotion activities of the Practice
- To develop skills in sexual health, including cervical cytology and sexual health screening
- To participate in chronic disease management within Practice guidelines
- To make referrals to, and receive referrals from other health care professionals
- To demonstrate a level of knowledge clinical skill appropriate to the needs of the role
- To be involved in the management of change within the Practice and the implementation of systems to improve patient care
- To participate in, and contribute to, the clinical governance agenda of the Practice
- To assist the Practice in achieving the Quality and Outcome Framework (GMS contract)
- To evaluate and audit care
- To participate in multidisciplinary protocol development
- To be responsible for the maintenance of supplies and equipment

Essential
- NMC Registered Nurse
- Driving License to travel to carry out home visit if required.
- Experience in Primary Care
- Experience working as a team
- NMC registered

- Previous Experience in primary care
- Experience in Systmone although training will be given
