We are seeking a motivated and organised Care at Home Coordinator to support the effective delivery and continuous improvement of our Care at Home service. The postholder will play a key role in coordinating staff deployment, responsibility for scheduling and live monitoring across Care at Home, managing referrals, monitoring performance and quality, and ensuring services are delivered in line with statutory requirements, policies, and procedures.
As Care at Home Coordinator, you will:
- Screen, prioritise and allocate referrals to Care at Home staff, ensuring appropriate and effective deployment.
- Be responsible for the scheduling of Care at Home care within the tool CM2000 by ensuring all service users are allocated Home Carers to provide their care.
- Be responsible for live monitoring the service by ensuring Home Carers are clocking in and out of visits.
- Participate in case file audit activity to ensure compliance with case recording and quality standards.
- Performance manage processes to meet local improvement targets and statutory performance indicators.
- Support the ongoing development and quality monitoring of the service, driving continuous improvement.
- Respond to and undertake complaints and investigations relating to service delivery and staff conduct, making recommendations for resolution where appropriate.
- Monitor service delivery, identify trends and report on service quality issues.
- Undertake absence management processes in line with Council policies.
- Ensure all services are delivered in accordance with policies, procedures and best practice.
- Line manage Care at Home staff, including training, support, performance management and implementation of Council policies and procedures.
- Support managers with Care inspectorate activities and other service priorities.
Experience required:
- Experience of working within health and social care services, ideally Care at Home or a similar setting.
- Knowledge of assessment, care planning and statutory performance requirements.
- Experience coordinating staff, managing workloads and meeting performance targets.
- Experience dealing with complaints, quality issues or audits.
- The ability to monitor performance data and identify service improvements.
- Confidence in managing staff and applying HR policies, including absence management.

Flexible work pattern – required to work weekends/evenings. The Scheduling team work between 7.30am – 10pm, 7 days a week, therefore there is a requirement for postholder to be able to work weekends/evenings to cover supervisions and carry out duties of role. Will be required to join an on-call managers rota for escalation purposes.
