We are looking to recruit a full‑time practice nurse at a large GP practice based in West Derby, Liverpool. Our multidisciplinary team includes a lead nurse, HCA, Senior Clinical Pharmacist, GPs, Physiotherapists, Social Prescribing link workers and 7 training doctors on site.
We are recognised for high QOF and Local Quality Contract achievement and we are committed to continuing our success through team growth. The role is supported by a Care Navigation and data team and an experienced management team.

Job summary
You will work alongside the lead nurse providing agreed lead areas of care, working autonomously and within an enhanced primary care team.
Primary key responsibilities
- Deliver and manage all cytology programmes, adhering to national guidance
- Ensure nursing participation within all quality contracts (QOF/GP Spec)
- Assess patient needs, incorporating evidence‑based practice
- Develop, implement and embed health promotion and wellbeing programmes
- Implement and evaluate treatment plans for chronic disease patients
- Identify, manage and support patients at risk of long‑term conditions
