To support the Health & Well Being Divisional Manager in the assessment that need to be completed on people who use our Health and Well Being African Caribbean Day Care Service and African Caribbean & BME Carers Information Advice and Navigation services. Within the Day Care service, the post holder will work with other members of the team if and when require providing practical, emotional and personal care support if and when required support so that we provide a seamless and person centred service and keep clients people folders up to date and information relevant and accurate.
Assessments and reviews can be carried out at within our venue or in people/carers own homes. The role also includes phoning people is they are unwell and visiting them in a hospital and intermediate care setting.
A key part of the role is around care navigation and ensuring that the person who uses our services have a formal assessment by Adult Social Care and they access local services that can maintain or improve their person and physical health which includes self-care.
The post holder will be expected to deal with enquires and referral relating to our services that come
from Community Professionals such as Social Workers, Clinicians, Social Prescribers, and people
who work in the wider Voluntary and Community Enterprise Sectors (VCSE ) and signpost to other
services where appropriate, to enable the individual to live independently in their own home and
prevent hospital re-admission.
To work in partnership with other organisations to effectively deliver a practical, flexible, coordinated, and accessible service understanding the needs of local community members. Day Care & Informal Family Carers Assessor will encourage and support individuals to access opportunities that will enable them to increase their confidence, resilience and help them to maintain their independence.
Day Care & Informal Family Carers Assessor will undertake duties required to meet the service user’s needs and to attend events, exhibitions and visit other organisations to promote our services and wider services that we deliver as an organisation and charity
