The Role

As a Care Practitioner, you will play a vital role in delivering high-quality care and support to individuals accessing the Adult Social Care Service at Hungerford Resource Centre and you will work collaboratively with a team of dedicated professionals to ensure the needs and aspirations of service users are met.

Your key responsibilities will include:

  • Providing direct care and support to individuals, ensuring their physical, emotional, and social needs are addressed.
  • Implementing care plans and monitoring progress, making adjustments as necessary to ensure optimal outcomes.
  • Advising and supporting individuals and their families regarding available services, resources, and options.
  • Collaborating with other professionals, such as healthcare providers and social workers, to ensure coordinated and holistic care.

  • Excellent communication and interpersonal skills, with the ability to build positive relationships with service users, families, and colleagues.

You will also be entitled to a generous annual leave entitlement starting at 27 days per annum (plus public holidays) rising by an additional 5 days after 5 years’ service.
