- Undertake home visits for patients as scheduled and triaged by the practice team.
- Perform comprehensive assessments, including history-taking, physical examination, and clinical observations.
- Make autonomous decisions regarding patient care, including treatment plans and escalation where necessary.
- Administer medications and treatments within scope of practice.
- Provide urgent care and stabilization for acutely unwell patients prior to hospital transfer if required.
- Record accurate and contemporaneous notes using electronic clinical systems.
- Support chronic disease management and preventative care during visits.
- Liaise with GPs, nurses, and other healthcare professionals to ensure continuity of care.
- Collect data for audit and quality improvement purposes.
