Healthcare Assistant (Frailty & Dementia) - Stour Surgery
We are looking for an enthusiastic and compassionate Healthcare Assistant with a special interest in dementia and frailty to join our friendly team at Stour Surgery.
This is an exciting opportunity to work within our Stour Community Outreach Team (SCOT). The Healthcare Assistant will play an important role in supporting some of our most vulnerable patients to remain safe, well and independent in their own home.
About the role
Working as part of SCOT, you will provide a mixture of planned and proactive care to our housebound and frail patients. This role will include:
- Undertaking annual reviews and health assessments for patients with frail and long-term conditions
- Completing frailty assessments and supporting personalised care planning
- Taking and recording clinical observations
- Undertaking blood tests for housebound patients
- Supporting dementia and frailty care, including working with patients, families and carers
- Identifying and escalating safeguarding concerns
- Identifying changes in patients’ health, wellbeing or social circumstances and escalating concerns appropriately
- Supporting care coordination and liaising with GPs, Nurses, community teams and other professionals
- Providing health promotion and preventative care
- Working collaboratively with the wider practice team to improve outcomes for our most vulnerable patients.
About you
We are looking for someone who:
- Is an experienced, confident and compassionate Healthcare Assistant
- Has a genuine interest in frailty and dementia (training can be provided)
- Can work confidently and independently in patients’ homes
- Has good clinical assessment and communication skills
- Understands the importance of safeguarding and person-centred care
- Is organised, pro-active and able to work as part of a multidisciplinary team
- Is keen to develop their skills and contribute to ideas to the continued development of SCOT
- Is competent and confident in taking blood samples
