We are looking for an enthusiastic Practice Nurse to join our friendly and supportive team at Welton Family Health Centre. This is a fixed term maternity cover position for 26 hours per week, expected to end September 2027. We are dedicated to the growth of our clinicians, offering training and opportunities to expand clinical skills. Regular debriefs, clinical discussions, and team meetings ensure a supportive and connected working environment.

Main duties of the job:

  • To provide high quality nursing care to meet the needs of the practice population, including treatment, screening, preventative care, patient education, and clinical quality systems maintenance.
  • To work in partnership with other clinical, management, and administrative colleagues to meet stated practice developmental goals.
  • To undertake cervical cytology sampling, immunisations as outlined by national programmes and local guidelines.
  • To provide support to GPs in the management of chronic diseases such as Diabetes, COPD, Asthma, and Hypertension.
  • To support the annual flu campaign and other mass population vaccination programmes as needed.
  • To act as a chaperone or similar support for other colleagues as needed.
  • To conduct and supervise other colleagues in anthropometric measurements, blood pressure, urinalysis, phlebotomy, microbiology samples, and other practice-based tests.
  • To carry out health checks in conjunction with national, local, and practice initiatives.
  • To provide education to patients on practice and local health services, identifying patients at risk of adverse health behaviours.
  • To promote the health and wellbeing of patients by counselling them on their conditions, medications, diet, exercise, and other health behaviours.
  • To participate in clinical audit in partnership with other clinical and administrative colleagues.
  • To stay updated with the latest developments in nursing practice, attending training sessions, workshops, and conferences.

Person Specification

Qualifications

  • Essential: RGN Qualification, Current NMC registration, Certificate in General Practice Nursing, Minimum 3 years post registration experience, Minimum 2 years General Practice Nursing experience, Clinical skills including cervical cytology, immunisation, and vaccinations.
  • Desirable: Accredited training in chronic disease management.

Experience

  • Essential: Experience of nurse-led management of long-term conditions.
