Homeless Health Link Manager Reporting to: Director of Operations Salary: £39,287.00 per annum Location: Twickenham, London Hours: Full-time, 37.5 hours per week (Monday to Friday) (Fixed-term contract to March 2026) About the Role This role involves managing and delivering high-quality services through the Homeless Health Link Service, supporting individuals experiencing homelessness, including rough sleepers, to access primary care, mental health, and substance misuse services. You will work closely with statutory and non-statutory agencies to improve pathways and outcomes for clients with complex needs while overseeing a team dedicated to this mission. Key Responsibilities Service Delivery * Build relationships with NHS Integrated Care Boards, local authorities, substance misuse services, and mental health providers to enhance service delivery. * Ensure staff conduct comprehensive assessments of referred clients, including risk and engagement plans for those with complex mental health needs. * Develop relationships with health services, homelessness services, and housing providers to facilitate effective client referrals and support pathways. * Provide advocacy for clients facing barriers in accessing services, ensuring their needs are met through coordinated efforts with local authorities and service providers. Resource Management * Provide leadership, supervision, and line management for staff, ensuring effective service delivery. * Promote the service across the community, building professional relationships with stakeholders and raising awareness of the available support. * Maintain up-to-date knowledge of resources for clients, signposting and referring them as necessary. Casework Management * Lead regular case reviews, working closely with partner agencies to monitor progress and address challenges. * Maintain accurate data and case management records in systems such as CHAIN and Inform. * Work collaboratively with clients to develop holistic, person-centered care plans that align with individual goals. * Ensure high standards of recording and reporting, including risk assessments, safeguarding alerts, and incident reports. Organizational Responsibilities * Support the delivery of objectives aligned with contractual KPIs and work collaboratively with funders and health partners. * Participate in team meetings, supervision sessions, and organizational training to maintain alignment with policies and values. * Maintain a safe and appropriate environment for staff and service users in compliance with health and safety standards. Knowledge, Skills & Experience Essential Experience * Proven experience in line-managing and supervising teams in homelessness, substance misuse, or mental health services. * Strong knowledge of services available to homeless individuals, including substance misuse and mental health support. * Experience in working with clients displaying challenging behaviors or complex needs. * A solid understanding of homelessness causes and pathways, alongside key legislation related to welfare benefits and housing law. * Strong organizational, communication, and case management skills, with proficiency in IT systems and databases. Desirable Experience * Experience working within the NHS. * Familiarity with psychological approaches such as IAPT and motivational interviewing.
