Salary: £32,750 - £35,564 FTE per annum (Pro-rata for part-time hours: £26,111 - £28,355)

Hours: 29.5 hours across 4 days – Part-time

Responsibilities

  • Referrals
    • Act as the main point of contact for referrals into BBBC’s youth social prescribing service.
    • Work closely with BBBC’s and the borough wide social prescribing team to generate referrals.
    • Build on existing referral pathways and proactively develop new relationships with youth services, schools and healthcare providers.
  • Personalised Support
    • Provide tailored non‑judgmental and inclusive one‑to‑one support to young people, focusing on what matters to them.
    • Co‑develop tailored action plans with young people to achieve their goals.
    • Take a holistic, youth‑centred approach, addressing social determinants of health such as mental health, education, employability and social welfare advice.
    • Refer and signpost young people to appropriate support services according to their needs.
    • Develop, manage and deliver innovative approaches to supporting young people, building on existing service delivery.
    • Manage and prioritise a caseload, in accordance with needs, priorities and any urgent support required.
  • Group Support
    • Co‑design and co‑deliver workshops and group activities addressing key challenges identified by young people such as mental health, self‑esteem, employability and social welfare.
  • Advocacy
    • Advocate on behalf of young people, liaising with schools, families and service providers to remove barriers and ensure access to opportunities.
    • Encourage young people to become active participants in their communities, fostering belonging and purpose.
  • Monitoring and Reporting
    • Ensure accurate data entry across internal and external systems.
    • Manage and collate data for reporting purposes.
    • Use data to identify and track unmet needs and inform service design.
    • Support service impact evaluation.

Qualifications

  • A recognised professional qualification in youth work or extensive experience with young people.
  • Appointment is subject to satisfactory Disclosure and Barring Service – Enhanced DBS.
