Procare is seeking an experienced Band 6 Senior Community Nurse to join our Night Nursing Service, providing high-quality care to patients in their own homes out of hours across Guildford and Waverley. You will support individuals with complex health needs and end-of-life care, conducting autonomous assessments and triage.
You will work with GPs, hospices, and community services to prevent hospital admissions, document with EMIS, mentor colleagues, and contribute to service development.
