Community Link Worker (Adult) in Shrewsbury

Hours:

30 hours per week. (Job share would be considered).

Period of employment:

Fixed term contract until 31st March 2027

Job Purpose

We are looking for a passionate and motivated individual to join our team as a Community Link Worker, with a focus on Adult Carers service delivery; completing registrations, attending external community events and growing connections with harder-to-reach communities.

• Provide a single point of community-based solutions to people with adult social care needs;
• Increase the visibility of the voluntary and community sector to operational staff within multi agency settings;
• To work in a multiagency way across all TWCVS projects and Adult Social Care, Children & Families Services, Public Health, Midlands Partnership Foundation Trust, other VSCE teams, stakeholders and partners.
• Provide information, advice and assisted access to services for members of the community;
• Support people to access digital platforms and technological aids for self-help and care;
• Providing an innovative and creative approach supporting people to live well locally.

Main Duties and Responsibilities

• To provide information, advice, support, and signposting for Unpaid Family Carers.
• Support Carers and respond to queries via telephone helpline and email inbox.
• Respond to referrals and self-referrals into the All Age Carers Centre from both the public and professionals, assessing if they are an Unpaid Family Carer.
• Work alongside internal and external colleagues to ensure a multiagency approach.
• Contribute content to quarterly newsletters, website, social media and other communication channels.
• Engage with members of the public at community hubs, meetings and events, offering practical, community-based solutions.

Community response & Engagement

• Responding to members of the public with a range of solutions including those available in community settings for members of the public/family carers at settings such as the Live Well Community Hubs across the borough.
• Engage, listen and build relationships with people of all ages, genders and backgrounds.
• Support and motivate individuals to develop solutions to the issues they care about to improve their health and wellbeing.
• Build and establish connections with harder to reach communities, breaking down barriers to access support.
• Generate income and donations to support the Adult Carer service provision.

Carer Support

• Deliver wellbeing activities, group sessions, and 1:1 drop-ins (across the community).
• Provide outcome-focussed casework for Adult Carers, ensuring their needs are addressed holistically.
• Support the individual on their caring journey and transition out of their caring role back into their community.
• Support with external references into services such as booking Carers Assessments. ·

Monitoring & Evaluation

• Maintain accurate records of all work undertaken, providing regular reports;
• Pro-actively monitor and evaluate work to measuring impact and gathering feedback on satisfaction.
• Collating feedback and views to inform future provision including case studies, quarterly report and presenting information to the Carers Partnership Board.

The successful applicant may from time to time be asked to carry out other duties within the organisation in line with the requirements of the CVS.

Telford & Wrekin CVS operates a friendly, informal team. However, we aim to maintain the highest standards of service and efficiency. Experience of working in a small team in a busy office environment would, therefore, be an advantage.

Support, supervision and training, where necessary, will be provided.

Qualifications / Experience

Evidence of a solid general education is required together with GCSE A*-C / 9-4 or equivalent including English and Mathematics.Developed keyboard skills and the ability to use Microsoft Office applications, i.e. Word, Excel, Outlook, Internet Explorer and an understanding or knowledge of Access are also essential.

A relevant professional qualification NVQ Level 3 Health and Social Care, or equivalent.

Evidence of further education or training that would support your application.

Hours of Work

Normal office hours:

Monday – Thursday: 9.00 am - 5.00 pm, Friday: 9.00am – 4.00pm

“Out of Hours” work and a flexible working pattern will be necessary to include working evenings and weekends.

Time off in lieu can be arranged (overtime is not paid).

The postholder will be required to undertake an Enhanced Disclosure and Barring Service check.
