Key Responsibilities ✔️ Lead and review Pathway Plans ✔️ Support young people with accommodation, finances, benefits and healthcare ✔️ Provide consistent guidance and emotional support ✔️ Coordinate with social workers, housing, education and health services ✔️ Advocate for young people and their best interests ✔️ Maintain regular contact and monitor wellbeing ✔️ Keep accurate and up-to-date case records
