Job Purpose
We are seeking an experiencedSocial Worker Level 3to join an Adult Social Care service. The successful candidate will be responsible for managing a varied and complex caseload, undertaking statutory social work duties and supporting adults to achieve positive outcomes while promoting independence, choice and wellbeing.
You will work within a strengths-based approach, working collaboratively with individuals, families, carers and partner agencies to assess needs, manage risk and develop appropriate support plans.
Key Responsibilities

Manage a caseload of adults with a range of care and support needs.

UndertakeCare Act assessments, reviews and reassessments.

Develop person-centred support and care plans that promote independence and wellbeing.

Complete safeguarding enquiries and take appropriate action where adults may be at risk of abuse or neglect.

UndertakeMental Capacity Assessmentsand contribute to Best Interests decision-making where required.

Work in accordance with relevant legislation, policies and professional standards.

Identify and manage risks while ensuring the individual's views, wishes and desired outcomes remain central to the assessment process.

Work collaboratively with health professionals, carers, families and other partner agencies.

Support hospital discharge and effective transition between services where required.

Make appropriate referrals and signpost individuals to community and voluntary services.

Maintain accurate and timely case records and complete assessments and reviews within required timescales.

Participate in multidisciplinary meetings and contribute to effective information sharing.

Promote least-restrictive, strengths-based and preventative approaches.

Manage complex situations and make sound professional decisions in the best interests of the individual.
Essential Requirements

Social Work qualification andSocial Work England registration.

Relevant experience working withinAdult Social Care.

Strong knowledge of theCare Act 2014and adult safeguarding procedures.

Experience undertaking assessments, reviews and care planning.

Experience of managing a complex adult social care caseload.

Knowledge and understanding ofMental Capacity Actprinciples.

Ability to work effectively with individuals, families, carers and multi-disciplinary professionals.

Strong written and verbal communication skills.

Ability to work independently and manage competing priorities.

Commitment to strengths-based and person-centred social work practice.
Desirable Experience

Experience working with complex or high-risk cases.

Experience within community adult social care or hospital discharge services.

Experience of safeguarding adults.

Experience undertaking Mental Capacity Assessments and Best Interests decisions.

Previous local authority or statutory social work experience.
