We Do Social Work is seeking a dedicated Leaving Care Worker to support young people aged 16–25 as they transition from care into independent adulthood. You will build trusting relationships, coordinating tailored plans with families, peer networks and partner agencies to ensure plans are delivered and opportunities accessed.
The role focuses on helping young people develop independence, maintain accommodation, access education, training and employment, and achieve positive wellbeing.
