We are currently seeking an experienced, motivated, and highly skilled Occupational Therapist to join our Adult Social Care team on a full-time basis. This is an excellent opportunity for an Occupational Therapist with strong local authority experience who is confident working autonomously and managing a complex caseload within a fast-paced adult social care setting.
As part of the Adult Social Care team, you will undertake comprehensive Occupational Therapy assessments for adults aged 18+ with disabilities, supporting individuals to maximise independence and achieve safe, person-centred outcomes. You will play a key role in assessing needs, identifying risks, recommending equipment and adaptations, and contributing to effective multi‑agency working across health, housing, and social care services.
Key Responsibilities- Undertake Care Act‑compliant Occupational Therapy assessments for adults with disabilities
- Apply sound clinical reasoning to assess risk, promote independence, and support safe discharge and community living
- Prescribe simple and complex equipment, including completion of associated risk assessments
- Complete Manual Handling assessments and provide safe moving and handling recommendations
- Assess for and recommend major adaptations in line with statutory eligibility criteria
- Refer for and contribute to Disabled Facilities Grant (DFG) processes where appropriate
- Participate in duty triaging on a rota basis, including screening referrals, responding to urgent matters, and prioritising cases effectively
- Maintain accurate, timely, and professional case recordings
- Work collaboratively with service users, carers, health professionals, housing teams, and partner agencies
- Degree or Diploma in Occupational Therapy
- Current HCPC registration
- Substantial recent experience within Adult Social Care in a local authority setting
- Demonstrable experience completing: Care Act assessments, Risk assessments, Manual Handling assessments, Simple and complex equipment provision
- Strong working knowledge of: Major adaptation eligibility criteria, Disabled Facilities Grant (DFG) processes, Equipment provision criteria
- Ability to independently manage a complex caseload
- Full‑time availability is essential
