We are currently recruiting for six experienced Social Workers to join a fast-paced Adult Social Care service. We are looking for confident, experienced practitioners who can manage complex caseloads, work independently and deliver high-quality, person-centred assessments and reviews.
We have opportunities across three specialist areas:
- Savings Project Social Workers – 2 Positions
- Undertake reviews, assessments and reassessments of individuals receiving care and support.
- Identify appropriate care and support solutions while considering opportunities for efficiencies and savings.
- Review existing packages of care and identify whether they remain appropriate and proportionate.
- Work closely with individuals, families, providers and other professionals.
- Complete accurate assessments, reviews and recording within required timescales.
- Make sound professional judgements based on risk, need and available resources.
- Manage a demanding caseload and meet project deadlines.
- Contribute to achieving agreed savings and service improvement objectives.
- Learning Disability Social Workers – 2 Positions
- Undertake Care Act assessments and reviews for adults with learning disabilities.
- Complete complex assessments and developing appropriate care and support plans.
- Apply the principles of the Mental Capacity Act 2005 in practice.
- Complete mental capacity assessments and best-interest decision-making.
- Manage safeguarding concerns and identifying and managing risk.
- Work with families, advocates, providers and multi-disciplinary professionals.
- Support individuals with complex needs to achieve positive outcomes.
- Prepare high-quality reports and documentation for legal and court proceedings where required.
- Work in accordance with relevant legislation, statutory guidance and local policies.
- Older People / Review & Reassessment Social Workers – 2 Positions
- Undertake Care Act assessments, reviews and reassessments.
- Review existing care and support packages to ensure they remain appropriate to the individual's needs.
- Identify changes in circumstances, risks and support requirements.
- Work with individuals, families, carers and other professionals to develop person-centred plans.
- Consider alternatives and community-based support where appropriate.
- Complete safeguarding assessments and risk management where required.
- Liaise with care providers, health professionals and other partner agencies.
- Ensure accurate and timely recording of assessments, reviews and decisions.
- Manage a caseload effectively and work to statutory and service timescales.
About You
For all positions, you will need:
- A recognised Social Work qualification and current Social Work England registration.
- Significant experience working within Adult Social Care.
- Strong knowledge of the Care Act 2014 and relevant adult social care legislation.
- Experience undertaking assessments, reviews and care planning.
- The ability to manage a caseload independently in a fast-paced environment.
- Excellent assessment, analytical and recording skills.
- Strong communication and partnership-working skills.
- The ability to make sound professional decisions and manage risk effectively.
- A commitment to person-centred practice and achieving positive outcomes for adults.
