Mental Health Social Worker – Hospital Discharge & Community Lincolnshire | Adult Social Care | This is an excellent opportunity for an experienced practitioner who enjoys working in a fast-paced, multidisciplinary environment and wants to make a real difference by supporting adults to leave hospital safely and successfully, while also providing ongoing support within the community. 📍 Location: Lincolnshire Service: Adult Social Care / Mental Health Focus: Hospital Discharge & Community Experience: Minimum 2 years' post-qualification Registration: Social Work England Driving licence & vehicle: Essential The Role You will manage a caseload of adults with mental health needs, supporting them through hospital discharge pathways as well as undertaking community-based assessments, reviews and care planning. Your responsibilities will include: Completing Care Act assessments and reviews. Managing a community caseload of adults with mental health needs. Supporting timely and effective discharge from inpatient mental health settings. Developing person-centred care and support plans. Coordinating appropriate care packages and community services. Identifying strengths, promoting independence and maximising people's choice and control. Assessing and managing risk to ensure safe discharge arrangements. Responding appropriately to safeguarding concerns. Reviewing care and support packages to ensure they remain appropriate and effective. Working closely with individuals, families, carers and support networks. Maintaining accurate and up-to-date case records in line with statutory requirements. Working collaboratively with NHS colleagues, care providers and wider community services. Contributing to effective multi-agency discharge planning. Supporting individuals to achieve positive outcomes and maintain their independence in the community. Hospital Discharge A key focus of the role will be helping adults transition safely from hospital back into the community. You will work alongside health and social care professionals to: Identify individuals' care and support needs. Coordinate appropriate services before discharge. Reduce unnecessary delays in hospital discharge. Ensure risks are identified and appropriately managed. Establish sustainable support arrangements within the community. Promote independence and prevent unnecessary readmission where possible. Community Caseload Alongside discharge work, you will hold a community caseload and undertake: New assessments Care Act assessments Reviews Care and support planning Risk assessments Safeguarding work Service and care package coordination What We're Looking For We are seeking conscientious, resilient and experienced Social Workers who have: A recognised Social Work qualification. Current Social Work England registration. Minimum 2 years' post-qualification experience. Experience working within mental health services, hospital discharge or integrated health and social care. Strong knowledge of the Care Act and adult social care practice. Experience undertaking assessments, reviews and care planning. Strong risk assessment and safeguarding skills. Experience working within a multidisciplinary/multi-agency environment. Excellent communication and relationship-building skills. The ability to work autonomously and manage a caseload effectively. Strong organisational and time-management skills. A commitment to delivering person-centred outcomes. Essential Requirement 🚗 A valid driving licence and access to a vehicle is essential.
