About the Role

We are currently seeking an experiencedMental Health Social Workerto join a dedicated team supporting adults experiencing a range of mental health needs.

The successful candidate will have strong knowledge and practical experience withinMental Health Social Workand will be confident working with individuals who may have complex and challenging needs.

Key Responsibilities

  • Manage a caseload of adults experiencing mental health difficulties.

  • Complete comprehensiveassessmentsof individuals' needs, strengths and risks.

  • Develop person-centred care and support plans that promote independence and wellbeing.

  • UndertakeMental Health Act and Mental Capacity Actrelated work where required.

  • Identify and respond appropriately to safeguarding concerns.

  • Work collaboratively with individuals, families, carers and other professionals.

  • Liaise effectively with health services, housing, voluntary organisations and other partner agencies.

  • Support individuals to access appropriate services and community resources.

  • Review care and support arrangements to ensure they continue to meet assessed needs.

  • Maintain accurate and timely case records in accordance with professional and organisational requirements.

  • Manage risk effectively and ensure appropriate safeguarding measures are in place.

  • Work in accordance with relevant legislation, policies and professional standards.

Essential Requirements

  • Qualified Social Worker with current professional registration.

  • Recent and demonstrable Mental Health Social Work experience – ESSENTIAL.

  • Experience working with adults with a range of mental health needs.

  • Strong assessment, care planning and risk management skills.

  • Understanding of safeguarding procedures.

  • Knowledge of theMental Capacity ActandMental Health Act.

  • Ability to manage a caseload effectively.

  • Excellent communication and interpersonal skills.

  • Ability to work collaboratively within a multidisciplinary environment.

  • Strong report-writing and recording skills.

  • Commitment to strengths-based, person-centred practice.
