We are recruiting an experienced and passionatePersonal Advisorto join theSupporting Independence Teamwithin Cheshire West and Chester.
This is an exciting opportunity to make a genuine difference to the lives of young people leaving care by providing thesupport, advice, guidance and encouragementthey need to develop their independence and thrive into adulthood.
The Role
As a Personal Advisor, you will provide anindividualised, outcome-focused serviceto young people who are legally defined as care leavers.

Build positive, trusting and professional relationships with care leavers.

Assess and understand their individual needs, strengths, aspirations and goals.

Work collaboratively with young people to developpathway plansand identify achievable next steps.

Provide practical advice, guidance and support to help young people live independently.

Support young people with areas such aseducation, employment, housing, finances, health and wellbeing.

Help care leavers develop the confidence, resilience and practical skills required for adulthood.

Advocate for young people and ensure theirvoice, wishes and aspirationsremain central to planning.

Work alongside other professionals and agencies to ensure young people receive appropriate support.

Monitor progress and review plans to ensure support remains focused on achieving positive outcomes.

Empower young people to make informed decisions and realise theirfull potential.

What Weβre Looking For
We are looking for someone who is:

Experienced in working withyoung people or care leavers.

Passionate about supporting young people to achieve independence.

Confident building positive and meaningful professional relationships.

Able to assess needs and developoutcome-focused support plans.

Knowledgeable about the challenges faced by young people transitioning from care to adulthood.

Able to advocate effectively for young people.
-
Organised and able to manage their own caseload.
-
Confident working with partner agencies and within a multi-agency environment.

A strong communicator with good written and verbal communication skills.
-
Resilient, compassionate and committed to achieving positive outcomes.
-
Able to travel throughout the local authority area.
-
A car driver with access to a vehicle.

Main office:The Portal, Ellesmere Port
Working pattern:Hybrid β combination of office and home working
Travel:Required across Cheshire West and Chester
