Manchester City Council is seeking a committed and skilled Adult Social Worker to join the Hospital Discharge to Assess (D2A) Team – South. This essential role supports patients at Wythenshawe Hospital and Trafford General Hospital, helping them achieve safe, timely, and well-coordinated discharges back home or into 24-hour care settings.
You will work closely with health professionals, community services, families, and external providers to ensure each patient receives the right level of support at the right time, promoting independence and preventing unnecessary delays in hospital.
Key Responsibilities- Undertake strengths-based assessments in line with the Care Act 2014
- Facilitate safe and timely hospital discharges using the Discharge to Assess model
- Coordinate support for individuals returning home or moving into short-term/long-term care
- Carry out Mental Capacity Assessments and Best Interest decisions when required
- Identify and respond to safeguarding concerns
- Work collaboratively with NHS colleagues, therapy teams, community services, and care providers
- Contribute to multi-disciplinary discharge planning
- Maintain accurate case records and documentation
- Qualified Social Worker registered with Social Work England
- Experience in hospital discharge, community adult social care, reablement, or similar settings
- Strong understanding of the Care Act, MCA, Safeguarding Adults, and CHC processes
- Able to work confidently in a fast-paced, multi-disciplinary environment
- Strong communication, assessment, and decision-making skills
