Via's Outreach service in Redbridge is seeking an Outreach Practitioner to provide assertive outreach and case management for people who are experiencing homelessness or are at risk of homelessness.
The role requires independence as well as strong teamwork.
You will perform triage and assessments, deliver psychosocial interventions, and work with partner agencies to connect individuals to wellbeing services and drug/alcohol treatment.
