Via Harrow is recruiting for a Dual Diagnosis Assertive Outreach Recovery Practitioner to join our Harrow service in the Borough of Harrow. The role focuses on delivering high-quality care to service users with complex needs, including opioid use and poly-substance use, and coordinating treatment across the borough.
You will work within a supportive multidisciplinary team, manage a caseload, and engage with external agencies to promote safe, person-centred care and recovery.
