Are you a Nurse or Pharmacist with an Independent Prescriber qualification and a passion for delivering evidence-based treatments in Substance Misuse? We are seeking dedicated professionals like you to join our team in Likewise, Sheffield and make a meaningful impact in people's lives, their families and communities. We are seeking a highly skilled and motivated Independent Prescriber to join our dynamic healthcare team. The ideal candidate will possess advanced clinical skills, a strong knowledge of pharmacology, and the ability to autonomously assess, diagnose, and manage patient care. This role involves collaborating with multidisciplinary teams to deliver exceptional patient outcomes and ensure the highest standards of care.

Main duties of the job

  • To deliver drug and alcohol clinical and prescribing services to the service population.
  • Clinical assessment with or without prescribing a range of medications (including controlled drugs) to support recovery in line with national and organisational guidelines and policies.
  • Prescribe cost effectively and according to best practice and provide prescribing information and advice to Pharmacists, Shared Care GPs and hospital staff.
  • Undertake Service User centred assessments, care planning, monitoring, evaluation and reviews in practice and outreach settings.
  • Blood Borne Virus (BBV) management (risk assessment, pre and post-test counselling, testing and immunisation).
  • Deliver health promotion and harm reduction interventions in a client-centred manner (e.g. safer sex, contraception, nutrition and healthy living).
  • Promote and deliver immunisation against Hepatitis B in line with national guidance.
  • Contribute to the maintenance of client clinical database; co-ordinate data entry and retrieval searches and reports to meet the internal and external service requirements as appropriate and ensuring data security at all times.
  • Liaise with pharmacists regarding supervised dispensing and prescription queries.
  • Share care with GPs for Service Users in Primary Care Extended Services (PCES)/Primary Care Recovery Services (PCRS).
  • Deliver clinical interventions across hubs in different locations and deliver outreach as required.
  • Undertake continuing professional development including participating in clinical supervision, performance appraisals and attending training as/when required.
  • Work in accordance with all relevant legislation, best practice policies & procedures and guidelines both internal and external.
  • Participate in the audit process and promote service wide or local clinical audit.
  • Ensure all serious incidents (including deaths in service) are reported, implementing any service changes arising.
  • Disseminate learning across the service and the organisation as appropriate.
  • Provide support and assistance to other Humankind substance misuse services during mobilisation.
  • Collaborate on clinical governance, review and implementation of policies and guidelines.
  • Assist in and have continued involvement with national and local research projects, as appropriate.
  • Keep abreast of policy and professional development within your area of professional expertise.
  • To promote, adhere to and live our workplace values of being honest, committed and inventive.

Person Specification

  • First level registered nurse or registered pharmacist.
  • RCGP Qualifications Level 1 (and 2 or willingness to work towards).
  • Proficient in Microsoft Outlook Office programmes.
  • Experience of entering complex information onto an electronic / clinical data base.
  • Experience in delivering evidence based treatments in Substance Misuse.
  • Previous experience of carrying out general health care assessment including BBV, and delivering health promotion/ harm reduction interventions, advice and information.
  • Awareness of social context of drug misuse and the wider implications of the behaviour.
  • Understanding of complexities of primary health care and the constrictions and opportunities inherent in the primary care sector.
  • Awareness and experience of the principles of Audit.
  • Understanding of drug misuse services available in the public and voluntary services.
  • Awareness of national strategies impacting on drug and alcohol services and role of Independent Prescriber.
  • Understanding of the differences between the private and the public sector roles and responsibilities.
