About the Role
To work as part of a team to deliver support packages to help individuals secure and maintain their accommodation, offering person-centred support, helping individuals to develop the skills and confidence to live independently. The role includes a mix of outreach work, covering duty within our office for individuals assessing our drop-in service.
Manage a caseload completing high quality and comprehensive needs assessments and risk assessments for individuals we support.
What You’ll Do
- Provide duty cover for individuals accessing our drop-in service.
- Manage a caseload, completing needs and risk assessments, developing and reviewing support plans.
- Work as part of a team as well as lone working, offering outreach support to individuals in their homes.
- Work with partnership agencies to ensure continuity of care and support.
- Promote safeguarding and the wellbeing of individuals we support.
To Succeed in This Role, You’ll Need:
- A real passion for supporting people, with some transferable skills, ideally from working in a similar environment or sector.
- NVQ Level 3 in Health & Social Care (or above or equivalent) or willing to work towards this in post.
- Basic IT skills and knowledge of how to use Microsoft Teams.
- Knowledge of housing options available and relevant legislation.
- An understanding of Safeguarding.
- Excellent communication, organisation and time management skills.
- A full UK Driving Licence.
