This is more than just a job its a chance to build meaningful relationships and create lasting change.About the Role As an Aquarius Practitioner , you will support individuals affected by alcohol and substance misuse by providing advice, assessments, and structured interventions.
Youll work with people on a one-to-one basis, deliver group programmes, and help develop personalised care plans that support recovery and wellbeing. The role involves working in a range of settings, including community locations and partner services, while collaborating with professionals and agencies to ensure individuals receive the right support at the right time.Key Duties Carry out screening, assessments, and brief interventions for individuals, families, or couples.Develop, implement, and review personalised care plans to support recovery goals.Deliver structured one-to-one support and facilitate group programmes.Provide guidance and support to individuals affected by someone elses substance use.Maintain accurate case records and monitor outcomes.Work collaboratively with external agencies and professionals to ensure integrated support.Promote the service through partnership working, events, and community engagement.Support and oversee volunteers or peer mentors where required.Ensure safeguarding procedures and confidentiality standards are always followed.About You You will have experience supporting individuals in health, social care, or substance misuse services and a genuine passion for helping people make positive changes. Youll be confident engaging with a wide range of individuals, able to build trusting relationships, and comfortable delivering both one-to-one and group interventions.You will also:Have a relevant qualification in health, social care, counselling, or a related field (or equivalent experience).Have knowledge of substance misuse, recovery approaches, and client-centred support.Be organised with strong record-keeping and IT skills.Be able to work independently while contributing positively to a team.Demonstrate strong communication skills and the ability to work with a wide range of partners and stakeholders.For Full Job Description Please Click HereWhat We OfferWe value the people who make a difference every day.
