We are looking for an experienced and motivated Practice Nurse with a strong interest and background in respiratory care to join our friendly and supportive primary care team. This is an exciting opportunity to work in a well-established practice committed to delivering high-quality and patient-centred care. The successful candidate will play a key role in the management of long-term conditions, particularly asthma and COPD, alongside providing general practice nursing services including cervical screening, immunisations, wound care and health promotion. Independent chronic disease management and respiratory qualifications are essential.

Main duties of the job
The main focus of this role will be the delivery of high-quality respiratory care, including the assessment, monitoring and management of patients with asthma and COPD. The successful candidate will lead respiratory reviews, support patients with self-management plans, optimise inhaler techniques and contribute to improving long-term condition outcomes within the practice. In addition to respiratory care, the Practice Nurse will provide a range of general practice nursing services, including chronic disease management, cervical cytology, immunisations, wound care, health promotion and screening activities. The role will involve working closely with the wider multidisciplinary team to deliver safe, effective and patient-centred care.
We are looking for an experienced practice nurse who can work independently, prioritise workload effectively and build strong relationships with patients and colleagues. Experience in primary care and respiratory is essential, and a proactive approach to service improvement and patient education would be highly valued.

Person Specification
- Evidence of continuing professional development.
- Experience working within primary care or general practice nursing.
- Experience using SystmOne.
- Experience participating in quality improvement or audit work.
Qualifications
- Registered General Nurse (RGN) with current Nursing and Midwifery Council registration.
- Respiratory qualification or diploma in asthma/COPD management.
- Competence in cervical cytology, immunisations and wound care.
- Spirometry qualification and interpretation experience.
- Travel health qualification and experience.
