Practice Nurse in Reading

Main duties of the job

• The successful candidate will provide a wide range of nursing services within the practice working both independently and as part of our multidisciplinary team.

• Desirably chronic disease management e.g. asthma, COPD, diabetes, hypertension
• Cervical cytology
• Childhood and adult immunisations
• Wound care and treatment room duties
• Health promotion and lifestyle advice
• Supporting patients with long-term condition self-management
• Participation in QOF and other clinical targets
• Contribution to clinical governance audits and service development

You will be expected to deliver evidence-based care, maintain accurate clinical records and work collaboratively with GPs, pharmacists, HCAs and administrative staff.

Job responsibilities

• To deliver high-quality, evidence-based nursing care to patients within the GP practice, working both independently and as part of the multidisciplinary team.
• Provide treatment room services including wound care, dressings, ear care, ECGs, blood pressure monitoring, and health checks.
• Deliver cervical cytology in line with national screening standards.
• Administer childhood and adult immunisations, ensuring safe storage, handling, and documentation of vaccines.
• Provide health promotion, lifestyle advice, and patient education.
• Support patients with long-term conditions, promoting self-management and adherence to care plans.
• Desirably undertake chronic disease management reviews (e.g., asthma, COPD, diabetes, hypertension).
• Perform clinical observations, assessments, and elevate concerns appropriately.
• Maintain accurate, timely, and comprehensive clinical records.
• Participate in QOF, enhanced services, and other clinical targets.
• Contribute to infection prevention and control processes within the practice.
• Work collaboratively with GPs, pharmacists, HCAs, and administrative staff to ensure coordinated patient care.
• Participate in clinical meetings, MDT discussions, and practice development activities.
• Communicate effectively with patients, carers, and colleagues, demonstrating empathy and professionalism.
• Ensure accurate coding and documentation of clinical activity.
• Assist with recall systems for screening, immunisations, and chronic disease reviews.
• Support the practice in meeting contractual and regulatory requirements.
• Uphold confidentiality and data protection standards at all times.

• Undertake any other duties appropriate to the role as requested by the practice management team.
• Work autonomously within professional boundaries while recognising when to seek support.

Person Specification

• Registered Nurse (NMC) with current PIN
• Cervical cytology qualification
• Up-to-date immunisation training (childhood and adult)
• Evidence of ongoing professional development and mandatory training
• Significant experience working in a GP practice or primary care setting
• Experience delivering treatment room duties (e.g., wound care, dressings, ECGs, health checks)
• Experience administering childhood and adult vaccinations
• Experience providing health promotion and lifestyle advice
• Strong clinical assessment and decision-making skills
• Ability to work autonomously within professional boundaries
• Excellent communication and interpersonal skills
• Strong organisational skills with the ability to prioritise workload
• Commitment to high quality, patient-centred care
• Understanding of safeguarding responsibilities for adults and children
• Ability to contribute to QOF, enhanced services, and clinical governance activities
• Experience maintaining accurate clinical records using electronic systems
