If so then this is an exciting opportunity for you to join the Community Reablement Team. We are looking for an individual who is passionate about working collaboratively with the people we support and colleagues to join us as a Reablement Officer and to support activity which maximises independence and wellbeing.

About the job:

As a Reablement Officer you will work with individuals in their own home to support and encourage them to develop or regain skills to live independent and fulfilling lives. You will work closely with the therapy team to deliver bespoke support and contribute to ongoing reviews.
