An exciting opportunity has arisen to join the Intermediate Care Service in Walsall, part of a multi-disciplinary team operated jointly between Walsall Healthcare NHS Trust and Walsall Adult Social Care. We are looking for enthusiastic and committed individuals to help shape and deliver our services, contributing to the model of best practice.

Successful candidates will have experience working in Adult Social Care and will join our multidisciplinary team. You will support customers in their own homes to develop or regain skills and confidence, coordinate and deliver a programme of active Reablement to support customers to achieve their full potential, working in accordance with individualised Reablement Plans. You will work cohesively within a multidisciplinary team to maximise customer independence and reduce the need for ongoing support, and support customers to enjoy full access to community resources and develop informal support networks.

Key Responsibilities
  • Support/enable activities of daily living (ADLs), including personal care, as part of a responsive service in partnership with other agencies, ensuring that religious, cultural and gender needs are met, and personal dignity and privacy are maintained.
  • Co-ordinate and deliver a programme of active Reablement to support customers to achieve their full potential, working in accordance with individualised Reablement Plans.
  • Work cohesively within a multidisciplinary team to maximise customer independence and reduce ongoing support needs.
  • Support customers to enjoy full access to community resources and develop informal support networks.
Qualifications

Minimum qualification: NVQ Level 2 Health and Social Care or equivalent. Excellent written and verbal communication skills and experience of community-based Reablement Services are desirable.
