An exciting opportunity has arisen to join the Intermediate Care Service in Walsall, a multi-disciplinary team operated jointly between Walsall Healthcare NHS Trust and Walsall Adult Social Care. We are particularly looking for enthusiastic and committed individuals to help shape and deliver our services and contribute to the model of best practice.
The successful candidates will be required to have experience of working in Adult Social Care and to join our Multi‐Disciplinary Team. You will support customers in their own homes to develop or regain skills and confidence, co‐ordinate and deliver a programme of active Reablement to support customers to achieve their full potential, working in accordance with individualised Reablement Plans.
Your responsibilities will include:
- Working cohesively within a multidisciplinary team to maximise customer independence and reduce the need for ongoing support.
- Supporting customers to enjoy full access to community resources and develop informal support networks.
Minimum qualification for this role: NVQ L2 Health and social care or equivalent.
