Walsall Council is seeking a Social Care Facilitator to join the Adult Social Care Mental Health Team. You will undertake assessments, support planning and reviews for adults with mental health needs, promoting independence and community inclusion.
You will work with Social Workers, AMHPs and partners to deliver coordinated support, ensure safeguarding, and apply the Care Act 2014 and Mental Health Act provisions in day-to-day practice.
