Walsall Community Network is seeking an ASYE Social Worker (Adult Intermediate Care) to support adults and their families, focusing on discharge planning, rehabilitation and safeguarding. The role works within a multi-disciplinary ICS team in partnership with Walsall Health Care NHS Trust and Walsall Council Adult Social Care.
You will carry out statutory duties under the Care Act and MCA and participate in a seven-day service with weekend/bank holiday rotations.
