We are seeking a dedicated and dynamic individual to join our multi-disciplinary team, supporting the delivery of the SEMH (social emotional mental health) Targeted Early Help Offer for families. This role involves working collaboratively with children, young people, and their families using a whole-family, strengths-based approach to reduce risk, build resilience, and improve outcomes.

As a practitioner for SEMH (social emotional mental health) Targeted Early Help, you will work closely with other agencies and directly with families to drive positive change. You will manage cases involving families with multiple needs, carrying out assessments, developing tailored support plans, and coordinating intensive multi-agency packages where required. You will also lead multi-agency reviews to ensure support remains effective and responsive.

The position is based in Castleford, co-located with Children’s Social Care, Health, Police and Employment advisors. Other professionals such as Education Improvement Teachers are also based within the building.
