Overview

We are looking to recruit a Care Co-ordinator post within our Adults Safeguarding Team, to enable us to improve our services to Adults and Older People within the Wakefield District. This is an opportunity to be part of a new and exciting challenge within the area of Adults Operations.

Responsibilities

  • Be a Champion within the Adult Safeguarding Team within the organisation.
  • Provide specialist advice and coordination.
  • Respond to concerns and make enquiries; work with the person subject to abuse.
  • Coordinate who will do what, e.g. criminal or disciplinary investigations.
  • Work closely with CQC.
  • Take responsibility for assessments and re-assessments, identifying where vulnerable adults may be at risk.
  • Be proactive in establishing eligibility, signposting to relevant services, assessing and determining the level of need to create outcome plans that reflect a person-centred approach.

You will be important in helping vulnerable people live as independently as possible. You will help identify risks, determine eligibility, signpost to relevant services, and develop outcome-focused care plans that reflect a person-centred approach.

Requirements and Standards

  • DBS (Disclosure & Barring Service): The successful applicant will be required to complete a Disclosure Application Form and provide criminal conviction information.
  • Fluency Duty: This post is subject to the Fluency Duty under Part 7 of the Immigration Act 2016. Demonstrate an appropriate level of fluency as detailed in the job specification.
  • Probation: In line with the Council\'s Probation Procedure, some positions may be subject to a six-month probationary period. This requirement does not apply to internal vacancies.
  • Diversity: We encourage applications from everyone and strive to ensure all staff feel included in the workplace.
  • Education/Knowledge: Not essential to have a social work degree; however, knowledge of care management and understanding of the Care Act 2014 is essential. It is important to be organized and to have current knowledge of relevant legislation.
