About the Role: We are looking for an enthusiastic and motivated Occupational Therapist to join our Hoki ki te Kāinga team which translates as Returning Home. This is a new role with an exciting opportunity to be based with our Hastings team while supporting the Central Hawkes Bay community three days a week. On the remaining two days you will be part of an interdisciplinary early supported discharge service that works in the community, supporting people in their transition home from hospital. In both roles you will play a vital role in supporting people to remain independent in their own home for longer.

Each day brings variety and the chance to make a tangible difference. You will complete collaborative goal setting with clients and their whānau, design individualised plans, provide equipment solutions, and carry out cognitive assessments. With a strong focus on maximising independence, you will turn everyday moments into opportunities for rehabilitation.

About the Team/Service/Location: Hoki ki te Kāinga is a community-based service that supports older adults as they transition home from hospital. Our focus is on providing the right support at the right time, enabling people to recover safely and regain independence. The Central Hawkes Bay component offers variety and the chance to expand your occupational therapy skill and embrace an interprofessional approach to healthcare delivery. This is an opportunity to work collaboratively with patients in the CHB inpatient ward and with clients in the community.

About you: We are looking for an occupational therapist who is:

  • NZ Registered Occupational Therapist with current annual practicing certificate.
  • Member of Occupational Therapy New Zealand - Professional Association (desirable).
  • Holds a Full and current drivers licence.
  • Enable NZ Accredited Assessor status, (or willing and able to attain this).

You will also need the following key attributes and skills:

  • Experience in rehabilitation, orthopaedic, geriatric, or community settings.
  • Strong clinical reasoning and assessment skills.
  • A passion for improving health outcomes and supporting independence.
  • Adaptable approach with a strong desire to help clients and whānau achieve their goals.
