# Needs Assessor – Registered Nurse/Allied Health Professional* **Hawkes Bay Fallen Soldiers Hospital, NASC*** **Permanent, Part-time 56 hours per fortnight (0.7 FTE)**Health New Zealand | Te Whatu Ora is firmly grounded in the principles of Te Tiriti o Waitangi and is committed to building a health system that serves all New Zealanders.**About the Role**We are seeking a registered health professional with a current practising certificate to join our Older Persons Needs Assessment Service Coordination (NASC) team. In this role, you will work closely with individuals, their whānau and support networks to understand needs, complete comprehensive assessments, and coordinate services that enable people to live well in their homes and communities.Youll play an important part in supporting older people and those living with long-term health conditions across Hawkes Bay, ensuring equitable access to the right care, at the right time.**About the Team/Service/ Location**Our NASC team is a supportive, multidisciplinary group committed to delivering high-quality, culturally safe and personcentred care. You will also demonstrate a commitment to equity and the principles of Te Tiriti o Waitangi.You will have:* A current practising certificate and at least two years experience in your relevant profession (RN or Allied Health).* Strong assessment, clinical reasoning, and documentation skills, ideally including experience with InterRAI.* Excellent communication skills and the ability to engage effectively with a wide range of people, including whānau and community providers.* The ability to work both autonomously and collaboratively within a multidisciplinary team in a busy environment.* Strong organisational skills, with the ability to prioritise, manage your workload, and maintain personal wellbeing in a demanding role.**Working at Health New Zealand**Health New Zealand | Te Whatu Ora is dedicated to ensuring excellent healthcare for the people of New Zealand.
