For over 150 years we have been at the forefront of changing vulnerable children and adults’ lives for the better. With significant changes happening in the social care sector over the next few years we are looking for enthusiastic and motivated individuals to join and support our teams.
Our Care@Home Service is currently recruiting a Full time 39 hours per week, Admissions Care Co-ordinator based at our Broomhill Activity Centre Office, participating in a 4 weekly rolling rota including on call duties and weekend work. The Admissions Care Co-ordinator Role involves coordinating the provision of quality services within VSA. Assessing the individual needs of clients, this involves liaising with all stakeholders and ensuring a smooth transition for clients entering the services.

Rate: Unqualified ÂŁ15.46, Qualified ÂŁ15.98

Main Duties/Responsibilities:
- Facilitate the smooth intake of clients seeking admission to the Care @ Home Services within VSA.
- Complete the initial screening for all referrals and to request and collate all information prior to admission.
- Work closely with the care co-ordinator to have knowledge of potential admissions and make yourself aware of the areas of need for rota purposes.
- Build and maintain positive relationships with referral partners by phone and email and in-person.
- Develop individualised support plans for clients in collaboration with clients' families and external bodies pre-admission, addressing specific needs, goals and aspirations.
- Oversee and participate in the maintenance of accurate recording systems and reports (both manual and computerized) to meet agreed standards in line with administrative, regulatory and management purposes.
- Monitor the provision of a safe and protective environment by complying with health, safety and security procedures, including medication administration, and reporting accidents/incidents when appropriate.
- Support service users in their daily living, according to their recognised and agreed needs, providing personal care where necessary, adhering to practice which promotes well-being, choice, giving people dignity, and treating them with respect.
- Encourage and support service users to develop skills and knowledge to live as independently as possible, if applicable, with reference to the risk assessment process.
- Support service users in developing and maintaining social networks and relationships.
- Provide the booklets and information about VSA to all new clients.
- Perform the role of key worker for the service user for a period of 6 weeks before handing over to the designated Team Leader.
- Comply with and have a sound working knowledge of necessary legislative requirements as laid down through the Health and Social Care Standards and recognising the impact of following these standards on the overall provision of service.
- Support service users in accessing and using services and facilities, assisting them to obtain advice in relation to financial matters if appropriate.
- Take the lead in planning, monitoring and reviewing service delivery plans to meet individual needs and preferences, within budgetary and funding constraints before handing over to the designated Team Leader.
- Be prepared to go out on support calls when required or in the case of an emergency.
- Contribute to the identification of the training and development needs of teams and individual staff members, taking appropriate action to implement plans to meet needs according to agreed procedures and guidelines.
- Help maintain an effective working environment by contributing towards the management of interpersonal conflict in working relationships.
- Assist in the implementation of disciplinary and grievance procedures where appropriate, according to VSA’s policies and procedures.
- Develop one’s knowledge and practice continuously through participation in training and development activities.
- Carry out other duties from time to time provided they are reasonable and within the post holder’s capabilities.
- Have awareness and work within internal control systems in your area to facilitate improvement in the quality of service provided.
- Basic IT Skills – Communication with Outlook is a large part of the role.

Qualifications/Training:
Essential:
- SCQF Level 6 in Social Services and Healthcare or other options as prescribed by SSSC
- Knowledge of SSSC Code of Practice
- Evidence of continued learning
Desirable:
- SCQF Level 7 in Social Services and Healthcare & ability and willingness to achieve 15 academic credits in Supervision and/or Management at SCQF Level 7.
- Knowledge of Health and Safety regulations
